Eligibility

Who is eligible to receive TCM services?

An individual who has a developmental disability that:

· Began before age 22

· Is likely to continue indefinitely

· Results in substantial limitations in at least two of the following areas

Living and working independently,

Written verbal communication

Self-direction

Learning

Mobility

Self-care

This eligibility determination will be made by Rolla Regional Office.  However, WCTCM can help with the intake process.  If you or someone you know would like to be considered for eligibility you may contact Amy Stringer at (573) 438-2866

Though eligibility for receiving case management services requires a person to have 2 of the substantial functional limitations, a separate standard is applied for the authorization of services through one of Missouri’s Medicaid Waivers. To qualify for services funded through a waiver, the person must have 3 substantial functional limitations from the list above.

Missouri currently has 4 Medicaid Waivers managed by the Department of Mental Health/Division of Developmental Disabilites.

Through these waivers, a wide range of service options exist and are determined by a needs-based assessment. (Not all services are available in all waivers)
